Indications
Mood disorders, complex regional pain syndrome, trigeminal neuralgia, PTSD, substance use disorders, and psychedelic therapeutic benefits to enhance neuroplasticity which enables integration of new healthy habits and beliefs.  
Age Group: 16 years and older
Exclusion criteria: active substance abuse (alcohol, cannabis, non-prescribed medications…), personal or family history of psychosis, pregnancy, history of increased intracranial pressure, uncontrolled hypertension, acute or unstable cardiovascular disease, and previous negative response to Ketamine

Administration of Ketamine
Our focus is on IV infusion, since it is one hundred percent bioavailable. We want to use the minimal effective dose, since we emphasize safety for all our patients. 
We start with 0.5mg/kg; as patients tolerate this dosing, we can slowly increase the dose to 0.6mg, 0.75mg, and 1mg/kg to maximize therapeutic effect of Ketamine. 
This IV infusion can be administered over 1-2 hours and can be stopped any time due to possible side effects and intolerance.  
Patient discharged with driver/caregiver once recovered to baseline sensorium and clinically stable

Cost is $400/infusion for a 4-8 week infusion cycle.   Some insurance does provide coverage for Ketamine treatment.  Some patients only need one cycles, while others may need treatment every 6 to 12 months.
